Respondent and ALL guests
Please read the following and acknowledged the COVID Policy for Event Participation by checking in the form on left:
An inherent risk of exposure to COVID-19 exists in any public place where people are present. COVID-19 is an extremely contagious disease that can lead to severe illness and death. According to the Centers for Disease Control and Prevention, senior citizens and guests with underlying medical conditions are especially vulnerable. Although All May See Foundation has instituted reasonable practices in an attempt to lessen or reduce the spread of COVID-19, All May See Foundation cannot guarantee that you will never get exposed to the virus or get sick from it. By attending an All May See Foundation event, you voluntarily assume all risks related to exposure to COVID-19.
We are requiring that guests be up-to-date on their vaccination. Up-to-date vaccination status is defined as meeting criteria in A or B:
Examples of individuals who are not up-to-date: have not completed the dose requirements of a COVID vaccine, or have not had the booster if eligible, or have not received an FDA or WHO-approved vaccine.
IMPORTANT: Guests who arrive at the event site without acceptable proof of full vaccination (vaccination card or digital record, e.g., CLEAR Health Pass app), as specified above, will NOT be allowed to enter.
Recommended
For the safety of all guests, we recommend pre-event testing by all attendees. (Pre-event testing clearance consists of a negative Color/PCR test within 48 hours, or a negative rapid antigen test within 12 hours before the event). Masking indoors is strongly recommended.
Health & Safety Information
Due to the fluidity of the current situation, additional specific safety protocols for All May See Foundation Anniversary Celebration may be announced closer to the event. All decisions will be based on best practices as recommended by the Centers for Disease Control and Prevention (CDC) and mandates from both the State of California and City of San Francisco. Updates will be posted to the event webpage at allmaysee.org/event.
